Does anyone know the name of this trick and where it can be found (book, DVD etc...)

This looks like a variation on Mike Powers' Holey Terror, or Holey Terror II (both commercial releases from the 80's), or the more recent Impromptu Terror (from his book Power Plays). It also spawned varations by Johnny Lindholm ("Impromptu Holey Terror" published in the Magical Arts Journal, vol. 1, #2) and David Acer ("No Holes Barred" published in Natural Selections). I'm sure that there are many other versions, and this one in particular (notable, I'd say, for the four hole punches versus two in the routines I have mentioned) will likely be identified by one of the more knowledgable Café members here.

I'm not sure but I would suspect Mike Power's Holy Terror II.
