Best thing I ever heard on this topic was a guy who had made a bunch of table tents made, one for each table. On one side it said "Yes Show Me the Magic" On the other "No thanks" or something to that effect. He would have the waitstaff ask the customers if they wanted to see the magician and position the table tent accordingly. That to me is just perfect and if I was ever to perform tableside magic I would absolutely use that.

The only problem is that they are making a decision based on not having information about what they are either seeing or turning down.

I work very closely with the wait staff and tip them accordingly. It's part of the venue I work for that my entertainment is optional but made available as a addon to the meal. After the wait staff has taken the orders they ask if while they wait or at some point after the meal they would like the resident entertainer to come to the table? If so I'm brought to the table at the appropriate time and introduced by the server. So no rejection or intrusion. And very professional as the customer know there will be an additional charge for my services. Understand that the menus make this clear. I don't work for tips but split the fees with the venue and the staff.
