Hi
I am currently rebuilding my website after it was hacked with an SQL injector and shut down. I am putting together a contact form, but I want to make sure I get all the information I need. Below is the list I have questions I have on the form so far and if their are any others I should add or delete or move around then I wold greatly appreciate the comments. First Name Last Name Address Postal Code City Province/State Phone Type of Show Date of Event Location of Event (Address) Time of Event Location of Event – Inside, Outside Age Range Expected Attendance How You Found Chris Comments Thanks Chris |

Here's the one I use. It just sends an e-mail alert to me and I contact the client from that.

Here's what I have on mine:
Name Company Phone Show Date Message I keep it very simple. I don't want to know EVERYTHING up front. I want to be able to call, to follow up and ask more questions and develop a rapport with the client. |

Yeah, simple is more effective. Long forms intimidate.

Chris, remember the long forms can scare them away because the viewer might not have the patience to give out so much information upfront. And the viewer might also think you are going to sell their information to a spam site because you are asking for so much.

Eric Paul, just had a feature that included this where I thought made sense and
is inline with what Scott mentioned above. Based on that, I would only ask for: Name/Company phone

Good point DynaMike - a form that is too long will cause some to shy away from filling it out at all. However, the best form will get you valuable info so that you can make a decent bid for service.

Another good thing to do, when using a form, is to ask for both home and work numbers, in addition to asking the best time of day to call.
