Has anyone felt any pain in the arm after performing the effect? After all we are physically doing something which our bodies are not used to. Sometimes I have a slight pain down my arm for a little while later

The best gimmick in my experience is to use no gimmick. starting and stopping can be almost instantaneous after some practice. I like to sometimes pretend I'm fainting while the pulse is stopped or become light headed. it adds a nice touch.
