Daniel Madison has one called Advocate. I like it.

Ted Lesley have an easy and workable index for 52 playing cards.

I believe Annemann's Practical Mental Magic, reprinted by Dover in 1983 and still in print, has a pretty good description of how to construct one. I never constructed it, but the drawings looked very good, and the description was very clear cut.

My favorite full-deck index is Harry Lorayne's "Self-Index" from Deck-Sterity. It just uses the deck--nothing else--and is super easy to learn and use.

IMHO it depends on what you are trying to do. Hofzinser's index worked pretty darn well.

Another vote for Daniel Madison's "advocate". This is a full deck index that is very easy to navigate. Smart thinking

Just Bought Daniel Madison's "The Advocate". I say... The best 20 bucks I have ever spent on a gimmick! This index is great!
