I believe Annemann's Practical Mental Magic, reprinted by Dover in 1983 and still in print, has a pretty good description of how to construct one. I never constructed it, but the drawings looked very good, and the description was very clear cut.

In a book by the Camirand Academy of magic in Canada, and I think authored by Gary Ouellet, Tony Binarelli, the flamboyant Italian magician had released the flattest possible card index. Really smart, simple and convenient.

My favorite full-deck index is Harry Lorayne's "Self-Index" from Deck-Sterity. It just uses the deck--nothing else--and is super easy to learn and use.

IMHO it depends on what you are trying to do. Hofzinser's index worked pretty darn well.

Just Bought Daniel Madison's "The Advocate". I say... The best 20 bucks I have ever spent on a gimmick! This index is great!
