I have a section on where they heard about me and any special instructions. I like the idea of estimated travel time.

Good Idea! It used to be more for marketing and gathering contact info. But it evolved into more information for myself. I staple the agreement letter and a map to the venue to this, and it's what I take to the event.

Mine was similar when I started off years ago. Now I have it in more of a professional manner. I just want to give you a few tips. 1). Shorten the length of lines that are not nesscesary. 2). Add a "CLIENT INFORMATION" heading also, because imagine doing a kid show for a business or public school. The contact person can be different from the client. 3). Add a line for parking/entrance fee. Sometimes you want the client to pay for parking/entrance fee also. 4). Add a "Estimated Audience" line. 5). Add a line for "SPECIAL INSTRUCTIONS." Something might come up different comparing to your other shows. 6). Add a "Paid" line. 7). Have more contact lines for the "Contact Information." For example: fax, cell phone, extension. 8). Change "Performance Address" to "Site Address." 9). Change "Show Phone" to "Site Phone." |

I used to have a whole section for client info, but I RARELY book myself directly with the client. I go through agents most of the time now. This is used as a quick overview of the show. More detailed info would be on the contract which accompanies this.

Even if using an agent (actually ESPECIALLY if using an agent) you should use the form anyway.
It cuts down on miscommunications. I use a form very similar but more of a contract than just an information sheet. I use a seperate booking sheet for that.

Mine looks pretty much the same except I have check off boxes for... Confirmation Mailed (That is my performance agreement), Date it was returned, Thank you letter mailed (Usually same day as show, one to parent and for me one to the birthday child) and Client added to mail list. You might consider these to be important as well. Frank
