You can make it yourself from wood, cardboard or foamboard following the instructions in "Winston Freer's Tile Deal Puzzle" found in the Mini-Mystery Book 1 at The Magic Nook. If you insist on buying it ready-made, here is one source:

Another one that is really good that works almost identical to the Winston Freer's Tile Deal Puzzle" is Puzzling Chocolate I like it better than Tenyo's Perpetual Puzzle.

Here is a variation of the new one I am making in my workshop from Baltic Birch. The difference is it has only one piece that can be added or removed and its 3.5 inches or square and .354 inch thick which is around 9 mm thick. The old version was 3.5inches x around 18 mm thick which makes this model half as thick.

Here is a photo of the new model.

The older version was sold through Stevens Magic Emporium a few years back and they may have one of what is now called the thicker version left in stock. Plus it wont break the bank like some of the other versions that run in to the hundreds of dollars. I get asked why don't you build the other version a lot and here is the only reason there is other guys building some very nice ones like the guy who makes a version in marble. So I see no reason to build the same thing as the other guys. Plus I wanted something a bit more simple and smaller for close up and this version fits that bill as I love to travel light.

For the last couple of years I was not able to make these as I lost my measurements. Then after seeing this I set back to make a new version but it had to be thinner. Which was a bit harder to do than working with the thicker wood. Surprisingly they still take a while to make and the finish takes a while too. The one in the above photo still needs another coat of finish buts its close enough to show. It was nice being in the workshop building something interesting again for a change.

Anyway just thought I show you an optional idea as they're a few out there that are really cool but sometimes hard to find.

One more thing with this one that is a bit different than one of the other models is that the starting position can be either or which is also nice. Meaning I can add the piece or subtract the center piece.
