I find that I almost always get 1 or 2 errors where two cards don't weave and it's usually somewhere in the middle.
Not sure how to correct this because I'm not even sure WHY this is happening...

If all the cards weave fine but a few in the middle don't, I would say it's most likely a problem with the cards you are using and their condition.

I don't know if anyone said this yet, but the pressure you're applying when weaving the cards together should be extremely gentle. This was something that held me back when learning, I was always trying to push the two halves together -- you instead need to treat them like they're going to phase through each other, only a very gentle downward pressure is needed.
