Can anyone tell me whether or not the book Carneycopia has a substantial amount of impromptu effects or is it mainly based on set-ups, gaffs, etc?

Honestly, I don't recall any gaffs used in the book at all. Same with set ups. It's all pure sleight of hand magic and one of my favorites!
